CONFERENCE REGISTRATION FORM
The full registration covers the conference sessions, the proceedings, coffee breaks, lunches, and the conference banquet. Student registration covers the conference sessions and proceedings. Only full time students are eligible. A proof of student identity is required. Cancellation and refund will not be entertained after registration. Substitutions will, however, be accepted.

Registration Procedure: Please fill in this registration form in block letters. Registration will not be complete until the payment has been received. Payments must be made in Hong Kong Dollars. We regret that payments in other currencies cannot be accepted. Personal cheques cannot be accepted. For payment by credit card, the form must be signed and returned by fax or post to: Ms. Josephine Wan, Department of Computing and Mathematics, Hong Kong Institute of Vocational Education (Tsing Yi), 20 Tsing Yi Road, Tsing Yi, Hong Kong.
